We will email you a history form 24-48 hours prior to your appointment. Please fill it out to the best of your ability and email it back to us. You can also print it out, take a picture and email it that way if preferable. If you can email it to us prior to your appointment, it eliminates a significant amount of your wait time. Also, if you are bringing in a new patient please email us any previous medical records prior to the day of your appointment.
