As the owner, you are still responsible for any charges incurred by the consent given by the alternate individual.
NOTE: For the safety of all animals here, it is our policy that all animals be up to date with their vaccinations in order to be hospitalized or boarded.
I, the undersigned, am at least 18 years of age and hereby state that I am the owner/caregiver of the above-specified animals.
The designation remains in place until the hospital is otherwise notified.

Some refills may require a visit prior to filling.
If there has been a change to your pet's medical status or your pet has not been seen in over 6 months, you may be required to set up an appointment prior to a prescription refill.
Greece Animal Hospital has a 24-hour policy on all medication refills.
If your prescription needs to be picked up on the same day, an additional $10 processing fee will apply.
Please Note: A Customer Service Representative will contact you, requesting payment, prior to mailing/shipping.
