Mountain View Veterinary Hospital is a new full-service companion animal hospital offering state of the art veterinary medicine to Canby, Oregon. Dr. Carrie Gartrell graduated from Oregon State University College of Veterinary Medicine and practices small animal medicine and surgery, preventative care, and behavior counseling.
She works with local obedience organizations and is actively seeking obedience and agility titles with her own pets.
Dr. Gartrell has an extensive background working with rabbits, and volunteers with local 4H groups.
She is currently a member of the Oregon Veterinary Medical Association, American Animal Hospital Association, American Veterinary Medical Association, and the American Veterinary Society of Animal Behavior.

Unpaid balances will be recovered as deemed appropriate and may incur a $30.00 administration fee.
A 1.5% monthly interest fee will be charged on all unpaid balances.

OPTION 1 - I prefer to be called before any additional procedures, other than emergencies.
If I cannot be reached, I authorize you to proceed with all necessary dental procedures not to exceed the amount of $500 or (fill in $ amount - You will be able to select your custom amount later) above the estimate.
I am aware that if the necessary procedure costs exceed this amount then the procedures will not be done.
A second dental procedure would need to be scheduled at a later date to finish necessary procedures.
