By checking "I agree", I, the CLIENT named above hereby Authorize the veterinarian to examine, prescribe for, and/or treat my pet(s) both listed above and unlisted.
I understand that unforeseeable adverse reactions to treatments are a possibility and the veterinarian is not at fault.
I assume full responsibility for all charges incurred for the care of this/these animal(s).
I also understand that these charges will be paid at the time of service, unless prior arrangements have been made.
Deliquent accounts are susceptible to late fees and/or submission to a collections agency.
Our veterinarians believe that prevention is the key to your pet's long-term health.
Pets age much faster than humans, so it's important to bring your pet in for an annual exam each year.
The wellness exam is similar to the yearly physical you receive from your doctor.
Our veterinarians will check the health of your pet from head to tail and will provide information and recommendations tailored to your pet's specific needs.
This visit will include a comprehensive physical exam and a time to discuss any changes in your pet's physical health or behavior.
It is important that we see your cat on a regular basis for a variety of reasons.
Even indoor cats can be exposed to infectious diseases and parasites like fleas.
Behavior problems are easier to treat if they are addressed when they begin, not months or years later.
It's important to know a cat's weight when he/she is healthy because weight loss is often the first sign of a problem.
If caught early, many geriatric diseases can be managed to give your cat a longer better quality life.
By getting to know you and your cat we can tailor our care to what you are comfortable with.
The interpretation of dog behaviors and preferred training methods have evolved over the years.
If your puppy or adult dog is exhibiting undesirable behaviors like anxiety or aggression it is best to seek the advice of a professional trainer or behaviorist.
There are veterinary specialists who can provide behavior modification techniques as well as appropriate medications to make your dog more receptive to training.
If your dog is aggressive or anxious during visits to the veterinarian there may be medications or other suggestions to help decrease his/her stress.
